C in Thailand:

@coelacanth∅... yes, made to order. For example, Thai tea or coffee could be ordered not sweet... usually with evaporated milk, but black as well. Condensed milk, sugar or syrup if sweetness is wanted. In Vietnam coffee could be ordered black (with sugar on the side, added by the customer). The condensed milk one usually at the bottom of the cup, and stirred in by the customer depending how much sweet they want it.

But sweet is still sort of the default, especially in the countryside. Like, occasionally, I might order not sweet, the preparer will repeat it but still start to put syrup in the beverage. For US, you should be able to order as well, as long it's not some instant mx. My wife and I do when we visit. You could order, for example, Vietnamese coffee black, not sweet, add your own milk.
